Allegiant Care Dental

Many of the dental plans offered by Allegiant Care are administered in-house by Allegiant Care staff. Our dental plans do not limit you to a specific network of providers.  Members and dependents may go to any dentist of their choice and eligible claims will be paid according to a fee schedule.

You may view the dental fee schedules for the current and prior years of the Forms page. The amounts you see on the fee schedule represent the maximum Allegiant Care will pay.  The patient is responsible for the difference between the provider’s charge and the allowed amount.  Dental benefit limitations and exclusions also apply.  If you have questions regarding dental, please contact the dental department at 1-800-258-9732 and selecting Option 2.
