Tobacco Avoidance & Wellness Program

According to the Centers for Disease Control and Prevention (CDC): “Cigarette smoking remains the leading cause of preventable disease and death in the United States. Cigarette smoking kills more than 480,000 Americans each year. In addition, smoking-related illness in the United States costs more than $300 billion a year, including nearly $170 billion in direct medical care for adults and $156 billion in lost productivity.”

The importance of every member and spouse getting an annual physical cannot be overstated. A physical examination helps your PCP determine the general status of your health, check your cholesterol, blood pressure, and blood sugar levels and treat minor conditions before they become severe. They also give you an opportunity to discuss important health issues like disease screening and lifestyle changes tailored to you.

Tobacco Avoidance & Wellness Program
Allegiant Care’s Tobacco Avoidance & Wellness Program provides a monetary incentive for members and their spouses who remain tobacco-free and who get an annual physical.

The incentive is available at two levels: $125 for member only and $200 for both member and spouse. (A spouse is not eligible for the incentive if the member does not qualify). A member and spouse (if applicable) must complete separate forms.

Incentive funds will be mailed early March.

2020 Tobacco Avoidance & Wellness Attestation Form

For Calendar Year 2020: Members who reported that they are tobacco-free during the open enrollment in November-December 2020 will have received a 2020 Tobacco Avoidance & Wellness Attestation form by mail in late January. Completed forms must be returned no later than February 15, 2021 to qualify for the incentive.

2021 Tobacco Avoidance & Wellness Attestation Form

For Calendar Years 2021: If you (and your spouse if applicable) are tobacco-free, you must bring the 2021 Tobacco Avoidance & Wellness Attestation form with you to your physical and have your PCP certify that you are tobacco-free.

To qualify for the monetary incentive, you must report that you are tobacco-free during the open enrollment in November-December 2021 and submit the 2021 Tobacco Avoidance & Wellness Attestation form(s) no later than January 2, 2022.
