Effective January 15, 2022, Allegiant Care will cover up to eight (8) At-Home COVID-19 tests per month for each covered member on your prescription plan. There are two options for coverage:

 

Option 1 – Preferred Retail Pharmacy (no out-of-pocket cost)

Visit the in-store pharmacy counter of one the pharmacies listed below and request an FDA/EAU approved at-home rapid test. You must provide your OptumRx ID card for payment. Approved tests are covered at 100%.

  • Rite Aid Pharmacy
  • Walgreens Pharmacy
  • Sam’s Club Pharmacy
  • Walmart Pharmacy

At-home COVID-19 tests may be not be available at all pharmacies due to nationwide shortages. We recommend contacting your local Rite Aid, Walmart or Sam’s Club pharmacy to confirm they are in stock.

Option 2 – Pay Up-Front and Submit a Reimbursement Request

Purchase FDA/EAU approved at-home COVID-19 tests from ANY retail pharmacy or online source and your plan will reimburse up to $12 per test, up to the monthly limit. You must submit a claim for reimbursement along with the receipt from the purchase. You will be responsible for any cost above $12 per test.

Claims for reimbursement must be sent to OptumRx. You may submit for reimbursement online or through the mail. You must submit a separate claim for each member of your family.

Are free tests still available from the government?

Yes! Every household in the U.S. is eligible to order a one-time shipment of four (4) free at-⁠home COVID-⁠19 test kits. The tests are completely free and do not require your insurance information. Orders usually ship in 7-12 days. If you haven’t already ordered your free tests from this source, you can do that now at covidtests.gov.

Which tests are covered by insurance?

Eligible at-home COVID-19 test kits must be authorized for emergency use by the Food and Drug Administration (FDA). You can find the FDA status printed on the outside of the box. Currently, the most common tests on the market include FlowFlex, Quickvue, Ellume, Binaxnow, Inteliswab and On/Go. These are the only brands covered at preferred retail pharmacies. Other FDA/EAU approved brands may be covered under the reimbursement option as long as the tests are FDA/EAU approved.

When can I submit my claim?

You can submit your claim now for covered tests purchased on or after January 15, 2022. Please submit your claim within (1) year of your purchase date. At-home tests will be covered through the end of the emergency health period.

Is there a limit to how many tests will be covered?

Yes. Each covered member is eligible for up to 8 individual COVID-19 tests per month. (e.g., a family of 4 would be eligible for 32 test kits per month. Note that tests may be packaged individually or with multiple tests in one package (for example, two tests packaged in one box). The monthly limit is 8 tests per covered individual per month, regardless of how they are packaged and distributed.

I purchased a test kit containing 2 tests. Will I be reimbursed for one test or two?

The $12 reimbursement limit is per test. If you purchased a single box with 2 tests you will receive reimbursement for 2 tests, up to $24.

If the preferred pharmacies near me do not have any tests in stock and I have to pay more to get them from another source, will my reimbursement still be limited to $12 per test?

Yes. All tests purchased from sources other than the retail pharmacies at Rite Aid, Walmart and Sam’s Club will be covered under the reimbursement option which is limited to $12 per test.

Can I order tests online from one of the preferred pharmacies?

You can order online from preferred pharmacies, but the reimbursement will be limited to $12 per test.
